Lack of familiarity: Most physicians who do not want to prescribe a medication off label don't prescribe because of a lack of familiarity or lack of comfort in using a particular medication. Need good research : "An open trial of naltrexone in the treatment of depersonalization disorder." was a small trial wi only 14 subjects, would not be sufficient to convince me that treating a patient wi Narcan for depersonalization is the best way to go. https://www.ncbi.nlm.nih.gov/pubmed/15876908 As a psychiatrist I would use standard treatments until there is strong research / evidence to support use of Narcan.
