Anal tag: Aside from the anal tag she might have an anal fissure as well that can be painful.

ANAL TAG/PAIN: Simple anal tag most of the time is harmless. Painful defecation can be because of constipation. If there is anal fissure (tear around anal opening) from constipation, the pain becomes so severe that the child becomes afraid of pooping and holds stool. Make sure she is not constipated. Anal tag may have association with other GI diseases. Check with her pediatrician to find out the cause.
