Concern: Would definitely recommend visit to rheumatologist. Need to tease out a good history and physical exam. Concern I have would be ankylosing spondylitis, which is an inflammatory arthritis that can affect the sacroiliac joint.

Many possibilities: Possible ankylosing spondylitis (as), but what's much more common is just simply pain in the sacroiliac joint from arthritic changes. An injection of corticosteroid into the si joint can help. Radiofrequency ablation of the si joint can help way longer. But b4 that, i give patients oral anti-inflammatories. Others: cluneal neuroma, local muscle pain, bertolotti's syndrome, iliolumbar, etc.
