Yes: By itself, an apoendicolith does not indicate appendicitis or a need for surgery. In medicine we need combined clinical findings and symptoms along with laboratory and radiology findings for a diagnosis of appendicitis. An appendicolith can be seen on x-ray ; be a normal finding for some.
