Spinal pain: Depends on the cause. You need to be evaluated. Assuming you've had mris you could have discogenic pain, facet pain or even sacroiliac joint dysfunction. All of these can be treated with various interventional procedures which can limit or even eliminate medications.

Always an option: Spine surgery has become very versatile. I think that there are many options for even the most refractory and challenging cases. I would need to evaluate your imaging studies such as mri, ct scan and x-rays as well as the physical exam. Never stop looking for the solution because it is definitely out there.
