Bacteria: The soft is generally plaque (bacteria) the hard is calculus.. Both are associated with periodontitis, inflammation, and bone loss. Without good foundation and bone, you can lose your teeth from mobility and perio infection. Routine professional cleanings and good homecare are paramount.

Dental 'scale': Calculus or "tartar" is the term for the hard calcium deposits that accumulate on the teeth. Like hard water scale in a coffee pot, calcium from saliva forms scale--binding to the enamel and salivary proteins of the mouth. It is normal to form tartar, but if not kept clean, the rough surface attracts bacteria (plaque) that can lead to gingivitis and gum disease. Your dentist can remove it easily.
