Pudendal Neuralgia: Assuming no infections or trauma, this sounds like pudendal neuralgia, since those are the major areas, plus the rectum, that are on the branches of the pudendal nerve which comes from the s2-s4 region. Look for someone experienced in diagnosing this and treating it. You might need a univeristy medical center.
