Egg effect is also a broad issue since a woman caries all her eggs in her ovary from birth, and there may be an effect from a variety of exposures that occur any day of her life. Numbers for individual defects are easier to come by than inclusive numbers for this reason. (ie.Cleft lip, down syndrome)  i've seen inclusive estimates from 3-6% for all bd's.

Varies: That varies tremendously.  Some are minor and don't need fixing.  Some are very severe and can't be fixed at all.  Many are in between;   they might not be completely fixable, but the right doctor might be able to make the problem considerably better.

Yes: Some can, but not all.  Taking Folic Acid supplements before conception can prevent spinal cord defects.  Excellent sugar control for a diabetic, avoiding certain drugs or medications early in pregnancy, and being aware of higher risks with advancing maternal age are all important.
