Relax! enjoy!: No big deal. This maybe normal for you! just relax and attempt pregnancy (if that is what you want). If the cycles become less frequent and more irregular, loose wt (if heavy). If still not getting pregnant for 1yr, then see a dr.

Your : Your wife has an ovulation problem.  If you want children now then she should see her ob/gyn or an infertility specialist.  They will give her medicine to help her ovulate.  This will also help regulate her period and reduce the risk of cysts.  When she does not want to get pregnant she should go on a birth control pill to pre vent cysts

Oral contraceptives: Sometimes oral contraceptive use can delay your menstrual cycle.  You will need to see a doctor and discuss with them starting ocps and how you can adjust your use to delay a cycle.

Assuming it is not: From atrophic vaginitis which means vaginal thinning due to lack of estrogen in the vaginal tissue - that can cause all of your symptoms, and can be treated with low dose vaginal estrogen like Estrace (estradiol) cream, if it is really from  episiotomy which is possible but not likely, the episiotomy can be redone with a minor surgery, you should go for a 2nd opinion before u would have that type of surgery.

Heat and pain killer: Use heating pad and non steroidal anti inflammatory meds. if still in lots of pain see an obgyn . After proper history and exam may be an ultrasound might be indicated. Oral contraceptives can be prescribed in some cases.

Thicker endometrium: When your periods are late there is more time for the estrogen from the ovaries to stimulate the enometrial lining to grow, resulting in more endometrial tissue to sluff off giving rise to more pain and more bleeding.

Options: Some of the more common treatments include:  ibuprofen, a prescription pain medication called ponstel, (mefenamic acid) and oral contraceptive pills are really good at helping to decrease menstrual cramps.
