Ok....: dermatologic conditions are hard to discuss without seeing them. There can be disorders of white patches needing more than BD (scar tissue, atrophic dermatitis, other). Obviously I cannot say wtihout seeing it directly, but a second opinion if it continues is one avenue.

See your PCP FOR DX: It seems we have lack of additional information like itching , burning or drainage from the lesion and sexual history. It look like fungal balanitis or yeast infection which need treatment with topical antifungal medicine.

Could be yeast: This could be anything from a yeast infection to mulliscum to penile pearly plaques I would recommend you see a dermatologist

: If it is smooth , not raised , or itchy observe it for a while. If there are any other changes you will have to be seen before a diagnosis can be made- by a Dermatologist or Urologist.

See Dr: Best to see a dermatologist or urologist. Balanitis of Zoon is usually much older men. Your description is not sufficient for diagnosis. An exam by a pro is required. No infection is likely to persist 3 years without more symptoms.

what can the patch be and how should i treat it?

Foreskin: This could be an early manifestation of a condition called lichen sclerosus which is usually itchy. If this is what it is, it can progress and eventually result in phimosis from scarring unless it's treated. I recommend consulting a dermatologist or urologist for proper diagnosis and treatment.

Red Patch on Penis: A red patch on the head of the penis can arise for several reasons.  If you are not circumcised, this could be inflammation, called balanitis. Steroid creams help.  If you have started any new meds, it could be a fixed drug eruption.  Stopping the meds helps, but it is a harmless event.
If it is growing in size and doesn't respond to the above in a week or two, i recommend a urological evaluation.

Balanitis, phimosis: This is due to a tiny amount of URINE trapped under the foreskin. Yeasts and smegma bacteria ferment the urine into irritating products. Circumcision is a quick and easy cure. Castellani  solution is very effective. Always hold back the foreskin during urination and hold back for a minute after. After urinating rinse off or blot with a tissue.
