In your belly: The kidneys are located in the retroperitoneum.  This is a fancy word for the back of your belly.  They lay on either side of your spine at about the level of the rib cage.

Your back pain: may be at the same level as your Kidneys,but that doesn't mean the pain is coming FROM your kidneys.It is much more likely to be back pain or possibly pleuritic chest pain ( from irritation of the lung lining).Seek medical attention if this pain persists.

Differnet things: There are several things that can cause this area to swell.  Some are more serious than others.   If your kidney is still working well based on lab tests then you should be evaluated for a hernia at the transplant sight.  Kidney that are undergoing rejection can swell.  Also other wound problems like old blood, fluid or leaking urine can build up and cause swelling.

Not likely: If you are having pain in your flank (kidney) area then it is highly unlikely to be related to your menstrual cycle as those body parts are not related. Schedule a primary care visit to look for kidney stones or kidney infection.

You should ask your doctor to further explain.

Depends.: Back pain may be caused by many different conditions, including kidney diseases.  If the kidneys are involved, it is usually because of an obstructing stone, inflamation, infection or reflux of urine.  Foreign bodies like  ureter stent often lead to pain.  Before labeling kidney pain, you should have appropriate history, physical and an imaging study to determine the location.
