Vulvitis: requires a GYN exam. You may have a fungal, bacterial or Barhtolin cyst infection. You can try antifungal cream like Ministat or Gyne-Lotrimin until seen; however, you must be seen and examined.

A sprain: You may have a ligament sprain and or a fracture. I would like to know if there is ecchymosis and or black and blue color. Also if the pain is over the bone and is it is a lateral bone or medial bone. Does the pain increase when you turn the foot inward or outward.

: Interesting!  You may have a dry mouth which can cause blocked salivary ducts.  This causes swelling which comes and goes.  I strongly suggest you go and get checked properly.

slipped on ice and landed really hard on the inner side of my knee, 3 weeks ago. bruising and swelling has gone down, but it burns every time it's touched. i can't kneel on it, this causes extreme burning/stinging sensations. will this go away?

Salt water rinses: You are experiencing a chemical burn from the whitening gel from an ill fitting bleaching tray.  Salt water rinses for the next 3 days will help or an application of ambulsol.  Discontinue tooth bleaching until the redness subsides.

The : The only thing that I am aware of that some people have found some relief with is l-lysine supplement. However i must say the most effective and reliable treatment is an antiviral like Acyclovir or valtrex (valacyclovir). Research low or no cost clinics in your area. Your local county hospital is a good place to start. Also try family planning clinics, there may be free programs that will cover a visit and medication for herpes. Lastly, you can research patient assistant programs for valtrex (valacyclovir). You may be able to get some help there. You may even consider calling your previous doctor for a prescription for acyclovir. Places like walmart and target offer these medications for very reasonable prices (i believe $4 for 30 pills).

Big toe: See your internist and have him check your uric acid levels to rule out gout.

X-ray needed: If you had work up done initially, a follow up will be needed to reassess. If you did not have it examined and based on Ottawa rules, an X-ray will be needed in order to rule out fracture.
