Can be normal: If you haven't missed any pills, then you are experiencing breakthrough bleeding. It is not dangerous and does not decrease the efficacy of the pill but if it happens frequently, change to a different formulation of pill, perhaps one with a higher estrogen level. About Plan B...: Plan B can alter the menstrual bleeding for the cycle in which Plan B was taken, and sometimes the next cycle too. One should be sure to check a home pregnancy test about 3 weeks after sex, because Plan B works most of the time, but not all the time. If cramps in the pelvis are "severe", a woman should call her Ob-Gyn doctor to see if an appointment is needed.

Miscarriage: if you had 2 positive pregnancy test followed by bleeding, you may have had a miscarriage. Talk to your GYN for further evaluation to undergo an US and /or beta hcg levels to determine if the pregnancy is viable or not.

skyla iud inserted 1/16/17. cramps/spotting (& brown discharge) haven't resolved. woke up due to severe cramping and pain. normal after iud insertion?

Can get checked: Bleeding and spotting is typical in the first month after and IUD insertion. The Progesterone in Skyla competes with the uterine lining leading to some spotting. Pain is not typical. Your pain may or may not be related to your IUD. The best thing to do is to schedule a follow up visit and have your doctor evaluate you.

Vaginismus: Dyspareunia aka Painful intercourse caused by vaginismus. For some women the vaginal muscles involuntarily or persistently contract when they attempt vaginal penetration. Vaginismus does not interfere with sexual arousal but it can prevent penetration. The pelvic and pap may be normal. 
There is treatment including Kegel exercises and vaginal dilators. Find gyn who specializes in pelvic floor d/o.

Yes: If the transfer was successful some implantation bleeding could be expected and the timing would be right at around 12 days.

This can occur: It is not unusual, but if it becomes heavier or greater volume and frequency it would be a good idea to see your obstetrician and have them examine you.  Good luck.
