Depends: Your return to a "normal" cycle will depend on your underlying cause of infertility.  For a woman who cycles monthly, it my only be a month or so, but for a woman with pcos it can be harder to predict.  Talk to your inferility doctor about what to expect after treatments and how to take care of yourself between attempts at pregnancy.

Soon: Usually within 3-5 days of stopping medications. Sometimes it can take a little longer. Periods is usually a little heavier than normal as well.

Unlikely: Its more likely that your hormonal levels are altered from the cycle and may cause this.

On the other hand, other possibilities include:

1. Undetected polyps
2. Rebound ovulation (earlier then usual)
3. Cervical trauma from the embryo transfer.

Couple preference: Should? Only if that's what the couple prefers. Most times it's personal rather than medical factors that make a couple delay, although medical considerations can sometimes make the decision. At age 38 6 months is not trivial, but likely not critical either.

Usually on time: Generally the cycle will come 2-5 days after the Progesterone has been discontinued   sometimes though the process may not happen as planned. At a minimum i would check another pregnancy test if the cycle doesn't start on time. Otherwise check with your provider for instructions. Good luck it's a tough process.

Doubtful: The spotting is most likely due to hormonal variations. Your GYN should be monitoring your female hormone levels, thyroid function and prolactin levels. Small cysts rarely cause bleeding.

Not much info: This is a controversal medication to use for fertility since its not available made by a commerical pharmaceutical company, you find it in health food stores or compounded. There are no good studies that show an improvement in pregnancy rates, and many mds around the country do not use it.

It takes time: Sometimes my patients will experience the symptoms of PMS but will not bleed.  I have seen this take from as little as one cycle to come back to as much as 4 or 5 cycles.  Be patient.  Be proactive, clean up your diet, have blood building meats like chicken and lamb. Get acupuncture. Acupuncturists can also create wonderful herbal remedies to boost red blood cells and get your period back on track

Soon: Most pregnancy tests are performed between 10 to 14 days after embryo transfer. If your menstruation doesn't start within the next 4 to 6 days after a negative initial pregnancy test, have another pregnancy test performed.
