Acute vs. Chronic: Acute otitis media is an acute illness with findings of inflammation of the mucosa of the middle ear as well as middle ear fluid. Chronic otitis media is diagnosed when there is a tympanic membrane perforation in the setting of chronic or recurrent ear infections, i.e. chronic purulent ear drainage despite appropriate antibiotic treatment.

ENT evaluation: You need to find an ENT you trust and follow his or her directions to the letter. Chronic sinus disease is difficult to treat and often combines multiple therapies including antibiotics and antibiotic nasal washes, steroids and/or steroid nasal sprays and sometimes surgical intervention. It does not turn around in 3-4 days . Usually takes 6-8 weeks of therapy to adequately treat.

When you fly or change elevation, the pressure difference increases, causing pain. Chewing gum or drinking and swallowing something may help.

Depends: If you have a perforation with drainage, sometimes just local antibiotic drops will resolve the problem.  If the drainage has a foul smell, you may have a cyst of skin called a cholesteatoma and that requires surgical removal.  Csom is a generic term.  Therefore, ask your otologist exactly what the problem is and why he/she thinks surgery is necessary.
