Unsure: Could be several things.  If it is severe or persists more than several hours then you should be evaluated.  Sorry but the list of possible causes is long.  With any type of abdominal/pelvic pain, best to be seen.  If any fevers, vomiting, diarrhea, sudden worsening of pain, please be seen right away.  Sorry this was not more helpful. Check urine.: This may be the beginning of a urinary bladder infection.  Increase your fluids, be sure of complete emptying of your bladder when you urinate.  I would suggest some cranberry juice daily.  Follow up with PCP for confirmation.

Primary care: General medical problems are best addressed first by your family doctor, usually a specialist in Family Medicine or Internal Medicine.  Many women see a specialist in Ob-Gyne as their primary care physician.  If the primary care doctor sees a reason to consult a specialist based on their evaluation or testing, they will send you to the appropriate doctors!

Referred pain: the pain at posterior superior iliac spine can be a referred pain from lumbar spine or pain comes from sacroiliac joint; pain comes from sacroiliac joint are typical related to the history of previous lumbar fusion or trauma;

Occipital mastoid : bone tenderness may indicate outer ear or occipital lymph nodes involvement if there's also mild swelling. If no swelling then it could be a normal finding, as long as there's no inflammation present locally around the area. Pain is important to discuss with your doctor. If mild tenderness only, watch that area, and call your doctor.If it doesn't improve, call your Doctor for further examination.
