Hard to say: An 11x12 mm follicle is either pre-or-post ovulatory, on CD 16 it could be either. If you check Progesterone level it will tell you for certain whether you ovulate or not.

Yes: So anything can happen. And actually the lining right before ovulation is much more indicative of implantation success, normally it is ideal to have the lining at least 8 mm thick, but 10-12 is much more common.

Ovulation : If the goal is to determine if you are ovulating and if you are fertile then other testing is needed. A mid luteal phase progesterone blood test can be done. This will give you much more useful information to guide fertility treatments

LUF: Luteinized Unruptured Follicle can sometimes happen. The follicle just does not rupture and there is no release of egg. Sometimes, elevated testosterone levels in PCOD can be associated with LUF. Please discuss with your doctor your options. It does not happen every cycle. so, please do not lose hope. Maybe next cycle will be easier and successful. Good Luck!
