Total hip: You will need to have a total hip replacement for the hip osteonecrosis.Osteoporosis need to be treated with the proper medicines(biphosphonates, orally o parentally.Parathyroid hormone subcutaneosly and vit d4000u daily.You will find out that after the total hip the back pain will improve and good pt program to strenghtening the core muscles will olso help with the back pain.

Intervertebral disc disease can cause muscle imbalance such that your posture and bone alignment changes, When this occurs the bones can respond by laying down more bone (called spurs) to offset your center of gravity. A thorough evaluation by a PMR Doctor might include MRI is appropriate. PT, Pilates may help.

Depends: Treatment is directed at what is the cause of the spur. Is it biomechanical: orthotics would be helpful. Is it due to a tight shoe: change shoes.  Is it due to a deformity of some kind. Then this should be addressed. Palliative treatment, including injections can be done. Surgery to remove spur can be performed if indicated.

If you are referring to heel spurs, then the treatments are very conservative for over 6 months before surgery becomes part of the discussion. Both have merits and cons that should be discussed with your surgeon. Disk replacement not for spurs.

Typical arthritis takes many more years to develop.  O-T-C anti inflammatory medication may help. However, long term use is usually not recommended. There are some topical anti inflammatory meds that can be prescribed. Best to see your Foot Specialist.
