Incomplete data: To properly label a 4 yo as a UTI you would need a clean catch or catheterized urine, and growth of &gt;50,000 colonies/unit on a properly plated culture media. Just having pee with a germ in it doesn&#x27;t qualify. The common squat pee collected in some offices is part vaginal wash out and will contain skin contaminant germs. This might or might not be a UTI.

Immediately: Uti is caused by bacteria multiplying in the urine. Thus bladder urine will be full of bacteria once you have a uti.

Inflammation. : Bacterial infections can cause inflammation/irritation of the lining of your urinary tract leading to traces of blood making its way in your urine. Also explains why sometimes with a bad cold we sometimes see streaks of blood in mucous and phlegm. Bacteria similarly cause irritation of the respiratory tract lining.

Mostly: Nitrite reagent in dipstick works for most gram negative organisms, but does show gram positive bacteria like enterococcus, staphlococcus or streptococcus etc.. Latter 2 are considered "skin" organisms but can be cause of utis in some, especially subjects on intermittent catheterization. Leucoocte esterase for wbcs in urine usually but, not always reliable. Urine culture is most reliable.

Urinalysis: Generally, we accept no more than 2 white cells in the macroscopic exam. More cells than that are abnormal and often reflect a urinary tract infection. I would definitely repeat the test (it is inexpensive) to make sure it was a "clean" catch, but if it is still abnormal, your provider will want to send a sample for culture to determine the proper antibiotic treatment.

Possibly: Urinary tract infections don't tend to be "occasional."  they tend to have a start and an end.  You should definitely have a urine test to see what is there especially at the time of symptoms.

Do the VCUG.: He should have the vcug. It is very important to find out if there is reflux of urine backwards, up toward the kidney, which will predispose to more infections. The ultrasound looks at the size and structure of the kidneys and urinary tract, but not reflux. You can not tell this from repeat urine tests either. Repeated infections can cause damage to the kidneys.

Genital infection?: Women often have elevated WBC in urine because of contamination of the sample with genital fluids. WBC are common in vaginal fluids, for example, often (but not always) because of infections like yeast and STDs. Also, symptoms of yeast and STDs often are similar to UTI. If you are sexually active and not tested recently for gonorrhea and chlamydia, make sure your doctor knows and get tested.
