One that helps you: Best is the one that helps you most. Each one responds different. I use a lot of combinations, mostly  Nucynta (tapentadol)  and Cymbalta  combination. I try to avoid narcotics, which are habit forming. Your doctor will guide you thru the choices.

Fibromyalgia: In general i often remind and teach my patient that this term, fibromyalgia, may not have a medical answer and that they should be looking at alternative medical practices and meditation and yoga as a way to manage living with this problem.

But, in general if you have not been put on Cymbalta (duloxetine) yet that would be the next step for me with medication.

Trigem neuralgia: There is no best pain med. Short term certainly stronger opioids may be tried but this is not a long term solution. You need to see someone about getting some nerve blocks etc.. Ultimately if nothing works then you have to consider a microvascular decompression of the nerve.

NSAIDs: I understand your trepidation. You may want to discuss nsaids with your physician. Thee are drugs like Ibuprofen and naproxen. Of course like all drugs they are not without side effects like GI ulcers, rectal bleeding.Etc. Talk it over with your physician.

A few things : Since shingles affects the nerves, i prefer anesthetics that numb those nerves.

Consider topical lidocaine. It's like topical novacaine (think dental anesthesia) 

talk to your doctor more about it.

Naproxen is better!: The equivalent dose of Ibuprofen 3200 mg is two 500mg of Naproxen (1000 mg total). It lasts twelve hours vs eight for ibuprofen!
