Genetics: Macular dystrophy is often cause by a defect in a gene for macular function, while age-related macular degeneration is not (currently known) to be caused by a gene malfunction, but from oxidative damage over time to the macula.

Essentially it's genetics vs environment.

Age of onset. : Typically, macular degeneration occurs in patients older than 55 years, and called age-related. There may be genetic associations as well. However, macular dystrophies are typically rare, inherited disorders occurring earlier in life which usually cause photoreceptor loss in the retina tissue. Both tend to cause central vision loss of varying degree. There is no effective treatment for dystrophy.

Many...: Macular degeneration usually occurs in older patients and affects primarily the central vision.  Night blindness can have genetic causes and is generally seen in children and younger adults.  If present, this should be checked especially if accompanied by hearing loss.  Retinitis pigmentosa is a common cause.

See Below: Dry (nonexudative) age-related macular degeneration (amd) is treated with areds (age-related eye disase study) vitamins. Wet (exudative) amd is treated with ocular injections (intravitreal avastin, lucentis, (ranibizumab) or eylea). Separate from all this, narrow angle glaucoma is treated with laser (peripheral iridotomy) and eye drops that help lower eye pressure (intraocular pressure).

Varries: I have seen patients with many different symptoms (first sign) of macular degeneration.  The most common is decreased ability to read (or need more light to read) or difficulty adapting to sudden changes in the lighting in the environment;  light to dark or dark to light situations.

Yes: The natural history of macular holes is that of continued enlargement, to a point, and associated progressive decline in visual acuity.  Additionally, there is progressive central distortion that evolves into a central blind spot.  Macular holes are best treated as soon as possible, though not urgent, and after about one year duration the prognosis for good vision drops dramatically.

Inherited: This is a condition, in several forms, which starts in early teen years and mostly progresses to complete loss of central macular function.  It is inherited and there is a strong family history.  Fortunately it is quite rare. The peripheral vision is usually preserved so a seeing eye dog is rarely needed.
