Site of healing: Here in kansas city, many (probably most) of us have little calcified granulomas from where we met and walled off histoplasmosis. Tb can do the same thing. It's little cause for concern; rarely these calcium nuggets get sharp edges and can scrape the surrounding structures.

left lateral neck hypoechoic lymph node 18x24mm. hyperaemic with effacement of fatty hilum. what exactly does this mean? thanks

Needs evaluation.: A completely normal lymph node will be less than 15mm and have a fatty center. When it enlarges and loses the fatty portion with increased blood flow (hyperemic), it should be sampled with a needle to make sure there is no infection or malignancy within it.

1.6 x .4 x 1 cm smoothly marginated lymph node without clearly defined fatty hilum found on left side neck during ultrasound. thats all in the report. what does this mean? whats the follow up? left zone iia?

: Talk to the doctor who ordered the test. It may be a normal node. Is it palpable? You may feel the nodes every couple of weeks, if any of the lymph glands doubles in size, becomes painful, ulcerates, or other nodes become enlarged, or you develop fever, or lose weight, you should see a doctor. Question? Diagnosis?: Not sure of your question, but I surmise you want a diagnosis (you want to know what this IS). Without symptoms &amp; physical exam (pictures), it is impossible to know. Even knowing symptoms &amp; appearance, sometimes the ultrasound is not definitive enough to give a diagnosis. See a surgeon for a biopsy; this may be a lipoma (benign fatty growth) or a lymphoma (benign or malignant, both types exist).

Unclear question: Why did you have these tests done? Do you have any symptoms? Did you discuss the results with the doctor who ordered the tests? Doing tests that are not addressed to answer a specific question often leads to other questions and unneeded investigation and treatment.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex.

Possible: The possibility of cancer is almost always there. The risk depends on the  smoking history, family history of cancer, the size of the mass, etc.. Smokers are at higher risk. Certain cancers are familial. Larger spots and those with ragged edges are more likely to be cancerous. Consultation with oncologist or lung specialist may be considered..

Probably benign node: When a mass is palpated in the neck it is usually abnormal and suggests a new, possibly  metastatic lesion requiring bx and then therapy.  If a hypoechoic mass is noted this is a solid lesion and with above description is a lymph node  that is characterized by the fatty hilum.  If lesion enlarges bx needed unless a primary lesion defined that may be causing nodal enlargement.  Therapy  then needed
