Communication: Between perineum and tunica vaginalis surrounding testis. Testes develop at back of abdominal cavity, just below kidneys. They migrate downwards and and out of abdomen through the inguinal canals attached to a slip of peritoneum, the tunica vaginalis at 27 wks. This normally closes after attached testis reaches base of scrotum. Fluid causing collection in scrotum via communication causes hydrocele.

Residual hydrocele: A residual hydrocele sac is the most likely cause. Your procedure was likely performed through a scrotal incision. 
In adults a hydrocele is a fluid collection around the testes confined to a potential space called the tunica vaginalis which when fills with fluid is call a hydrocele sac. The fluid and most of the sac are removed during the repair but if some sac remains recurrence may occur.

Not necessariiy: Most hydroceles are painless. They may look painful duto their size. They come in two flavors: communicating and noncommunicating. A communicating hydrocele changes in size as fluid goes to and fro betwenn abdominal compartment and the scrotum. A noncomunicating hydrocele is static in size and may resolve if it occurs at birth. Surgery will fix communicating hydrocele.

Scrotal ultrasound can evaluate testis if not palpable within a tense hydrocele.H, s seldom serios in boys.
