External/internal: A complete pelvic exam would typically include, after a history evaluation,  a careful visual inspection and digital exam with fingers and a speculum  - to accurately assess both the external and internal female vaginal, vulva, cervix, uterus, tubes, ovaries, and adj. Tissues.   A pap test is freq. Done, as well.   Occasionally, a sonogram is helpful.

Consultation: First get a consultation with your physician.  There are many factors that may need to be addressed.  A blood test is a start and then determination of a possible hormonal imbalance.  Your concern may be the initial entry into a problem that is affecting other parts of your body function for health that needs addressing.

Multiple causes: It could be infection, it could be hygiene issues, it could be a lack of fresh dry air to the vaginal area.  Vaginal ph (acid level) may also effect the odor slightly.  Recent unprotected sex will also cause an odor.  I recommend no underwear to bed when not on your period, shower over tub, cotton panties over thongs and non cotton, and wiping front to back as a start. See you gyn if odor is new.

Its possible: Genitalia development depends on  the chromosomes and exposure to   testosterone.  There is a sequence of events starting with the internal genitalia (ovaries, tubes, testes) that ultimately play a role in the development of the external genitalia (outer vagina or penis).  If a female, for ex.,  has all of her genitalia and has an enlarged clitoris (may look like a penis), she could conceive.

Unusual problem: When there are certain states of estrogen deficiency, one may experience dryness and chaffing that may appear similar to lacerations. Otherwise, I am not certain what could be the cause other than excessively tight garment wear  producing abrasions or trauma from other sources of manipulation.
I would advise consultation with your gynecologist.

Genitalia: That is an unusual question for a 45 year old woman - the development is much earlier in life and menopause will kick in over the next 5-10 years.

If you are a man looking for gender identity change make an appointment to see a Psychologist. Only if she/he agrees with your desire should you be referred to a clinician.

Most such patients I see are much younger than 45.

is it safe to put lidocaine on female genitalia? will is cause a numbing affect?

What for?: If a woman were to want to use a numbing cream because sex is painful, i'd want to look further into the reason. Many women in the 'perimenopause' will experience vaginal dryness as their estrogen production decreases. There are lubricants and hormone medications that can be helpful. I would be concerned that numbing the vagina might permit you to be injured without being aware of it! talk to doc.
