Pre or postnatal US?: Kidneys are occasionally generous in their size on a prenatal ultrasound and revert to normal by 2 wks. When this is detected in pregnancy, i generally do another study @ 2 wks if i will be following the child or after birth if they plan to move to another town or dr. A post natal study is more accurate & can define more precisely if there are issues like a backup of urine in a ureter.

Some of these problems can be minor, others can be life-threatening.  If you have been told that your baby's kidneys are enlarged, ask to be referred to a maternal fetal medicine specialist.

When both kidneys are enlarged this can occur related to blockage caused by kidney stones, or an enlarged prostate, diseases such as diabetes or polycystic kidney disease, inflammatory diseases etc. There are many reasons.

Several things:: Blockage of the ureter (e.g. Stones, tumors, valves) can lead to accumulation of fluid in the urinary space, leading to the most common form of enlargement called hydronephrosis. Severe urine reflux from the bladder can cause this as well. The organ itself can be enlarged in inflammation, such as a kidney infection; infiltration by cancer; or if there is a large or multiple kidney cysts.
