Anything: You can have sleep problems from snoring, apnea, restless legs, poor sleep hygeine, anxiety/depression, napping during the day, night seizures, noises in the neighborhood waking you up, bed bugs, waking up to use the bathroom, acid reflux, allergies, or any number of things. I recommend seeing a doctor to get checked out.

Sleep maintance ....: Need more details but looks like..Sleep maintance difficulty so keep same sleep wake cycle,  avoid caffeine in afternoon and any day time naps. Sleep restriction therapy and not to sleep in bed  or stay in bed , if cannot fall/stay  a sleep >15 min. Look for any medication side effect if on. Melatonin at bed time can be tried. Exercise regularly  and if constipated take supplements for regular  bm.

AM awakening: A couple of possible reasons. Insomnia due to early morning awakening can be a symptom of major depression.  It can also be related to anticipatory anxiety. For instance, someone who is afraid to be late to work may consistenly wake too early.

Sleep hygiene: Sleep hygiene involves going to bed at the same time every night as well as waking up at the same time every morning. Chances are, the earlier you go to bed in the evening the better chance you have of being able to wake up early in the morning, hopefully you will also feel refreshed.
