There are numerous reasons acl surgery fails and there are a variety of ways to treat that failure.  Revision surgery, while good, does not usually yield as good as a result as one gets after the 1st surgery.

Not really known: Mucoid degeneration of the ACL is seen most commonly in adults (35-55 years of age). The cause is not really known, but patients usually do not complain of instability. Anterior knee pain is the more common complaint. The ACL appears on MRI to be enlarged with cystic changes throughout, however it is intact. Arthroscopic debridement may help relieve the anterior knee pain.

Autograft: There are several choices for a graft for acl reconstruction: patellar tendon autograft, hamstring autograft, and allograft.  Studies show that taking an autograft in a young patient has a lower rate of re-tear as compared to an allograft, so that would be your best option.  Between hamstring and patellar tendon, the jury is still out, and i would go with the one your surgeon recommends.

Physical therapy: Physical may include dozens of different exercises including the two you mentioned.  Consulting with a physical therapist after surgery is critical to the healing process.

Necessary: After a severe injury to the knee like an ACL injury & subsequent surgery, the knee is weakened, swollen, painful, and in need of major healing.  Without thearpy, one may not gain all of the potential that is left in that injured knee.  It may be painful to go through the rehab, but it is necessary.  With the right therapy, supervision & medications, you should do well.  Work hard.  It's worth it!

Depends : If you had surgery, you should follow you'd doctors protocol.  If not you want to emphasize full motion, and straight ahead activities,  if your knee wobbles or locks it should be checked and surgery considered.

Deep in the middle: Deep in the middle obliquely connecting the thigh bone to the leg bone. It provides stability especially rotational stability required in pivoting movements.

No: The acl cannot "dislocated" per se and still be intact. If this occurs you would have an acl tear.
