No midwife: Most episiotomies are not needed.  If you are seeing a midwife your are less likely to get one.  Doctors get in a hurry and don't want to wait for mother nature.

Communication!: The idea that automatic episiotomy is the standard of care stopped being the norm about 2 years ago and many physicians have adopted this belief.  Make sure your doctor is on the same thought process as you!  another idea is "perineal massage", whereby your partner gently massages/stretches the bottom of the vaginal opening with a gel (k-y jelly) for the last 8 weeks in preparation for delivery.

Accepted Answer

No: It is not mandatory to have an episiotomy with every vaginal delivery.  Sometimes it is necessary to allow the baby to come out more easily or in an effort to prevent a larger laceration that would occur naturally.  Your doctor will decide on the need for one at the time of your delivery.

No: Scars in this area may need revision if the pain is such that it interrupts normal sexual intimacy.   Search out an experienced uro-gynecologist.

See below: Sometimes that is only way that the baby can delivered with a vaginal delivery.  The other alternative is a c-section which is more invasive.  Talk to your doctor about concerns and options.

ASK YOUR OB: Who delivered you, that is your ob's job to tell you all of that before u go home, just like i tell all of my pts so call their office today and follow their instructions as they know what type of epsiotomy u had and if you had any other problems down there.

Post-partum sex ?: As any surgical repair, an episiotomy will take from 4-6 weeks to heal sufficiently to withstand vaginal intercourse.  Even at that point, pain from the scar tissue may still occur.

Depends: May  have  worsening  pain,  foul smelling discharge, separation or  opening of  the wound.A few women will state that  the  stiches came out or broke.

Pelvic exam: It is not likely that you reinjured an old episiotomy unless the episiotomy was very recent. There are other causes of vaginal pain that should be evaluated.The next step is an examination by your Obgyn.

Sitz bath.: Frequent warm water sitz bath should be soothing. Make sure you do not have pus draining and you do not have fever, otherwise you should see your doctor. Antiseptic like salt may be helpful to reduce chances of infection. You may want to wash after you move your bowel rather than use toilet roll.
