Anxiety : Anxiety is a real problem and as you know it can affect your life in a major way. You need to tell your parents or guardian what is going on. Your pediatricain can refer you to a mental health specialist for an evaluation and treatment options. Treatment can be medication, therapy, or both. You, your provider, and parents will decide on the best plan for you. In addition you can discuss this with the school nurse who can also refer you for care.

Society anxiety: If their is a family member or a childhood friend allow them to help you meet others, particularly positive people.  
There are others there that may feel the same as you. But also remember, that you are never alone, physically or spiritually.

Fat also provides energy and satisfies the appetite more than do carbohydrates.

Difficult ?: There is no formal screening of athletes in the us. Coutries like italy have a mandatory screening for all athletes consisting of an ekg followed by further testing as needed. 
See your family doctor especially if there is a family history . A referral can be made to cardiology if there is a high suspicion.

Complete recovery: Return to play for a high school athlete should not be considered til all symptoms have resolved. It is important that the athlete have no symptoms during exercise as well. As the athlete heals, they will have no symptoms at rest but exertion can cause mild symptoms. If they are at this point, they should refrain from competing. If you have access to baseline neuro testing, a repeat test may help.

Sometimes: When a girl going through puberty begins having periods is often related to how much body fat she has. A very low percent body fat, or a very high level of intense physical activity, may delay the first period. You may also have other health problems if your first period is delayed too long. If no period by age 13 or 14, it would be wise to see a gynecologist.

Vision changes: Yes. It is normal for your vision (eye muscle function and lens transparency and flexibility) to change throughout life. Some experience changes in late gradeschool, highschool, college, and at any time beyond. It's basically a part of aging. It's just that we can never tell how fast it is going to progress.

The : The previous answer is true.  There are many potential sources of lower back pain.  I've have seen misdiagnosis very frequently and it is absolutely shocking how often it occurs given the frequency of lower back pain.  You have seen many doctors, but as you know, not all doctors are the same.  Even with the same board certification and training, they vary quite widely in their ability to differentiate between minutia.

I don't know if this answer creates more confusion for you, but i would highly recommend vetting out a true spine and pain specialist to see if something is being missed.  In any case, core strengthening is recommended.
