Effusion: Unclear. Fluid on the knee (effusion) can come from a number of causes and some times it will go away without treatment other times it won't.  The most common causes of fluid in the knee are an acute traumatic injury, meniscal tears and forms of arthritis or cartilage lesions.  Effusions from trauma can resolve on their own provided that they have not caused a meniscal tear or cartilage injury.

Aspiration: It depends on the cause of accumulation and amount and associated conditions. 
But if it is significant and painful and you are not taking any Blood thinner, at least for the symptomatic relief the fluid can be aspirated by your doctor.
 Continue takin Ibuprofen unless it is acute and Traumatic, then you need immediate attention to make sure there is no Blood. Apply Ice too. Rest.

Apply cold if recent injury, warm if chronic. Also r.I.C.E. (rest, immobilization, cold, elevation).

Go see your doctor: But first, there's supposed to be fluid in your knee. Synovial fluid acts as a shock absorber of sorts so you really don't want to get rid of it. Second, why do you want to mess around w/your knee if it doesn't hurt? Better to leave well enough alone. Bottom line, go see your family doctor and explain in full detail what's going on and what's on your mind that concerns you about your knee.

i have had xrays on my left knee to see what is wrong with it due to it being swollen and hurting back in july. i was diagnosed with have patella in both knees. after physical therapy, my swelling went down - but now its back when i am moving a lot. it doesnt hurt but there is a lot of fluid on the knee and it seems to go away when i work out. but swells when im doing daily activities: walking, etc. wondering if i need to go back to the doctor or continue pt exercises
