A fistula: A fistula for dialysis is created by connecting a vein in the arm to an artery and giving it usually 6-8 weeks to mature. A good fistula is anatomically not visible but felt through the thrill and bruit it creates. So you should not be able to know the location of a fistula if it is well done.

Lump on skin: Hemodialysis sites look like engorged veins.  If they are "fistulas" (artery connected to a vein) the lump on the skin follows the natural pathway of the vein. Over time these can dilate and become aneurysmal. The arterio-venous grafts are basically synthetic / plastic tubes which are placed right under the skin. These  you can see and feel. The actual needle puncture sites develop small scabs.

The A filters the blood of the patient removing waste products and water the diseased kidneys of the patient cannot do. This prevents the patient dying of kidney failure. The D does not affect the patient's kidneys.

Dialysis preparation: You should have a good idea of what type of dialysis you want, either hemodialysis (hd) or peritoneal dialysis (pd). Each has its advantages and disadvantages. Once you and your nephrologist have decided, have a av fistula created for hd or implant a peritoneal catheter for pd. The best thing you can do is to start dialysis without a central vein catheter. Speak to your nephrologist about this.
