Irritation: If the nerve roots are under pressure from a disc herniation or tightness from the spinal canal itself ( i.E stenosis), the nerve roots will be compressed and they become inflamed and swelling occurs. This is painful and the treatment can be exercises, injections and ultimatley surgery to relieve pressure if non surgical methods to not work.

With severe cases: pressure can cause many problems, as you suggest. Weight loss if needed and certain diuretic medications can help. Lumbar puncture is a short term solution and also helps in diagnosis.

Possible causes include, but are not limited to: rib contusion, rib fracture, costochondritis, shingles, and intercostal neuritis. I would seek a formal evaluation from a qualified medical professional - especially if your swelling is associated with chest pain and/or associated skin lesions.

Different items: Papilledema is a sign of increased intracranial pressure of any etiology, and can be due to tumor, hydrocephalus, brain hemorrhage, infections.  Swelling of the optic nerve can be due to optic neuritis or other inflammatory problems.  Spinal fluid leaks can be due to trauma over skull such as cribiform plate fracture.  If severe trauma, perhaps all could occur together.

: Yes, it would be unusual to have significantly elevated intracranial pressure in absence of papilloedema, and it is correct that a direct pressure measurement via LP is the most direct and objective test.  But maybe have an ophthalmologist double check eye grounds using a red free light to check maculopapillar bundle fibers, a subtle sign.  Chronic daily migraine  can be broken up with CGRP's.
