Balanitis?: It sounds like you have balanitis, the medical term for inflammation of the head of the penis and the foreskin, especially if in addition to "cuts" there is redness, whitish patches, or odor. It might be "nonspecific"; or possibly a yeast or bacterial infection. See a doctor. Once healed, prevention is usually just normal hygiene, i.e. retract foreskin and wash once or twice daily.

Foreskin: Tears of the foreskin usually occur if the foreskin is tight over the head of the penis,  tear from intercourse, infection. Diabetic patients can be prone to this problem. Usually antibiotic and steroids can help temporarily but the solution is usually a circumcision.

Diabetic: I think self-treating diabetis with non-prescribed Insulin is asking for trouble. You need to see a physician. Cuts in your foreskin need to be treated and you may need a circumcision.

See a urologist: Or dermatologist. No harm using miconazole but this is useful to clear yeast or fungus, not cuts. Why do you have cuts???? Sounds like you will better off after circumcision. This is a quick, easy procedure.

Tight foreskin: If you have difficulty retracting your foreskin or if there's often an itchy rash there, you should consult a urologist for evaluation,diagnosis and treatment. Early phimosis and/or lichen sclerosus would be concerns.
