Varies: There are a lot of possibilities and i would encourage you to get checked.  Many potential causes are musculoskeletal and quite benign, such as a subscapular bursitis or a thoracic radiculopathy.  But there are more potentially dangerous conditions including the possibility it is an abnormal presentation of cardiac ischemia.  A detailed exam and appropriate tests can usually determine the cause.

Could be a: pinched nerve in your NECK (C6-7)

Coughing: Yes, it would worthwhile to be seen to rule out pneumonia or a flu like syndrome

Lump near shoulder: There are a few possibilities.  If it is painful and no lump or nodule is found then it can muscle spasms causing a knot.  It could be a cyst or a lipoma which is a fat collection.  If the size of it is greater than 1 cm or it has grown recently or has become more painful or it is causing other symptoms like numbness then i would have it evaluated.

Many possible causes: The twitching may be related to any number of reasons including stress, stimulant overuse (for example, caffeine), overexercise, or nutritional deficiencies. Gallstone pain is typically precipitated from the ingestion of fatty foods, and is manifested as pain under the right rib cage and penetrating toward the back ("under your shoulder blade").  Gallstones are typically radiolucent and, therefore, need to be seen on US or CT scan.
