Menstrual pain: It sounds like your menstrual pain is radiating to the hip joint. You may want to have a pelvic exam with your gynaecologist if this is new, or more severe than usual. Pain, as stated above, though, can radiate and it seems to come with menstrual cycle so related to that.

Depends: Sometime the fluid can be posterior (in back of hip joint) so if you come in from the front, you may not get it.  Usually, you can though if there is a lot.  Also, the aspiration of the hip needs to be done with image guidance to make sure you're in the right spot.
