I agree with your: Endocrinologist. You have ideal body weight and if u have no other issues such as hypertension, diabetes and elevated lipids, this fatty tissue is benign. This is not fluid and this area is not a common location for fluid collection. People with cushing's (elevated adrenal steroids) develop 'buffalo hump' where swelling is in the upper back of the area you're describing. Take care!

Emergent Bochdalek?: Always go to the er if you have new, worrying symptoms and are concerned. Unlikely that the bochdalek hernia is causing choking. The pain and fullness is a possibility, although it could also be something else like gallbladder disease.

PND: What your describing could be several things..But the first thing that comes to mind is post nasal drip.  It can become quite thick and cause a lot of clear phlegm expectoration.  If you find yourself clearing your throat constantly..It may well be this.  You should see your pcp in case its something more severe.

Several: This could arise from dehydration (not drinking enough water), sinus drainage from allergy or infection, productive cough. If you are a smoker, lack of cilia activity to clear the lung can also cause this. Drink more water. Good luck

Isthmus: Should have TSH levels. Needle biopsy indicated if treating physician thinks it represents mass.
