First Aid Ointment: Not sure about what gauges are but Triple antibiotic ointment obtained at the pharmacy is helpful in many skin wounds. Scabs are healing skin areas. The ointment will help prevent infection, and redness, while moisturizing and protecting the skin.

Probably not: If the spot has really remained unchanged for years, then it likely is not serious. However if it's bothersome, a dermatologist can tell you what it is and likely take care of it. If the spot is changing, then it needs to be checked.

Anti fungal: I am guessing you had this diagnosed by an MD. You should have been given an RX for an anti-fungal.  You could try Clotrimazole, or Tolnaftate (cheap at dollar stores) applied 3 x per day.  If persistent see an ENT specialist or a dermatologist.

See a dermatologist: The ear can sustain sun damage. This can be anything from just a mild condition (actinic keratosis) to early skin cancer formation. See a dermatologist. Sometimes a specific prescription skin ointment can be used, or other options if needed.

I would recommend: that you have him examined to check those scabs. He may have impetigo, which is a bacterial infection that needs to be treated. The lumps behind his ears are probably inflamed lymph nodes that may or may not be related to the scalp lesions.

Skin infection : It sounds like impetigo.  Clean the area with soap and water regularly and use some antibiotic cream or ointment.  If it persists after doing that for a couple of says, then see a physician for an evaluation and probably stronger antibiotics.    Avoid putting your hair behind your ears until healed.
