Transient or ...: If the redness goes away in a few days, I won't be worrying over it. If it is persistent but does not itchy or hurt, I won't lose sleep over it either.  Do your eyes tear frequently?

can herpes cause balanitis for over 3 weeks? i tested negative for all stds and wa s just wondering. maybe i have a yeast infection from my girlfriend b/c she had thrush with no pain. i've noticed that i had a redness around forskin like ring shaped

Not herpes: No herpes outbreak is likely to last 3 weeks, it's an uncommon cause of balanitis. If herpes were the cause, it would be in the form of open sores, generally very painful. Yeast indeed can cause balanitis. Maybe share your gf's anti-yeast cream (or buy clotrimazole cream over the counter). Also retract foreskin and wash w/ soap and water 2-3 times a day. See a doctor if these don't help.

Allergies : Allergies cause red eyes and skin. Cold compress can relieve the swelling and redness.   Cetaphil or eucerin cream can help with dry skin around the eyes.  Over-the-counter allergy medicine, such as loratidine (claritin) or cetrizine (zyrtec)  should be taken to stop the overall allergy.  Over the counter eyedrops help relieve eye itching: zaditor (ketotifen) is a good eye drop for this.

Dryness: Licking your lips and letting them dry repeatedly can cause a red ring to form around the outside borders of your lips. Keep them moisturized and try not to lick them.

Get him evaluated: A puncture wound of this type can provide easy access for germs to the deep tissues/ligaments of the foot where a deep infection can arise and eventually require surgical drainage and hospital care. I would contact his doc and discuss the issue. He will likely need to see him to decide if this can be managed at home or needs immediate aggressive care.

Yes: Signs of inflamation like , surruonding redness, swelling, tenderness can indicate infection, even without drainage.

Irritation: Generally some redness around the remaining skin on a circumcised penis is a local irritation, with a local skin infection also a possibility (so many bacteria in the perineum!).  The best thing to do is use water to wash the area (no soap or wipes), and put some a and d or even a little antibacterial ointment on the area).
