No problem: A wax impaction does not harm a perforation.  As an ENT i suggest that all perforations be repaired via a tympanoplasty  or myringoplasty at some point.  An intact ear drum is safer and less likely to become impacted.  Patients who choose not to have a tympanoplasty  should have an annual ear exam by their doctor.

No: The sound arising from a piano can not have any effect on a perforated ear drum.  The perforated ear, however, might have an effect on your hearing when playing the piano.  The perforation can be repaired.  You can not harm your ear with the playing of the piano.

Yes...diluted 50-50: Yes, is the simple answer, If it is diluted 50-50 with water. But better if the PCP's office with ability to look into the ears can do the flushing.

See ENT: Sounds like you need to see an ENT specialists. Speak to your GP about a referral. Other than that keep it dry, no swimming and would use ear plug in shower.

See ENT: With 60% involvement, it is unlikely to heal up by itself. I would urge you to contact your ENT to see what he/she thinks.

Go to ENT.: It might be moot at this point, but an ENT doctor can take a look to see if it's even still ruptured at this point.  You can always decide from there, and the actual examination shouldn't hurt at all.

Ear drum repair: Some repairs can be done in the canal. Others require a behind the ear incision. The behind the ear incisions heal really well.  Very little of the hair will be shaved if need be.   The graft will be taken from the tissue behind the ear. It will heal nicely. Good luck.
