Many possibilities: This is much too complex for an online reply. Many possibilities and maybe more than one problem. Cushing's disease (excess cortisone production). Treatment with steroids, e.g. prednisone. Side effects of other drugs, e.g. antiretroviral drugs if you have HIV. Underactive thyroid gland. Others. Presumably you're seeing a doctor; keep working with her. Maybe endocrinology consultation. Good luck!

You will likley need further disgnostic studies such as blood work, perhaps x-rays or other imaging studies, maybe a biopsy or using a scope to check . Normalize hormones: Cushing's disease (presumably of pituitary origin) leads to excess cortisol secretion. This leads to hypertension, increased heart size, weight gain, diabetes mellitus, increased infection risk. With normalization of the cortisol levels, there is improvement in the blood pressure, blood glucose levels,  and weight loss. Although there may be decrease in fatty tissue, the stretch marks may remain.

Weight gain: Antihistamine, usually the older ones, may be associated with weight gain. Loratadine in the recommended dose usually does not cause it however. I have not heard of this causing bloating which is more likely from something else but this is another complicated subject.

You should have him get evaluated again for his weight to rule out metabolic problems like hyperthyroidism, gastrointestinal absorption problems or behavioral problems like anorexia or bulimia.

Not sure what you: Mean by 'powerful' advice.  If you have alcoholism, it is common to gain weight - alcohol has 8 calories per gram (about 120 per drink) and if you are drinking enough, you can take in 1000's of extra calories. It also inflames the stomach, liver and pancreas.  You need to stop, and most people need help stopping. Talk to your doctor about a referral to a treatment program and get help now.
