Orthotics: Improperly fitted orthotics can cause problems with the feet, knees, hips and back.  See a podiatrist or foot specialist if otc is not working, you may need custom ones.

Accepted Answer

Yes: Orthotics are primarily for comfort.  If they hurt, they are not helping you.  Some orthotics prescribed can make a condition worse such as if an orthotic with a high arch is given to a patient with a cavus foot.

Accepted Answer

At first: Basically, these are new prescription devices for your feet and your feet are now using some muscle groups that it hasn't used as the devices functionally correct the problem.  Typically, your physician should give you instructions to break-in the devices (eg. 2 hours, 4 hours, 6 hours, etc.) if it continues to bother you after 3 days of trying, call your doc.  You may need to have it adjusted.

Orthotics should : Not cause long term problems they should do the opposite. 
 I do not believe orthotics weaken muscles.

Support: An orthotic may properly support your arch which can be helpful if you are having pain.  The orthotic may also improve the biomechanics of your lower extremity.

Accepted Answer

Depends: First, we don't know whether you actually need orthotic devices or not, .  In my experience, many people who are prescribed orthoses either don't need them, or are prescribed the wrong ones.  Assuming you do need them, it is best to wean into them slowly, as it can take a while to get acclimated .  If they continue to hurt you need to get them adjusted, or get a second opinion.

Minimally: Off-the-shelf orthotics are minimally beneficial to some people. It is  best to have them made specifically for your feet.

See below: If by molded you mean custom orthotics made from a mold of your foot, then the answer is yes.

Can't really compare: They are both great specialties but focus on different aspects.  The DPT is a Doctor of Physical Therapy who will focus on restoring function and maintaining independence and in some cases the Orhtotist/Prosthetist creates the functional and structural devices that may be necessary to help do that.
