Epididymitis: is my first thought: an infection or inflammation of the epididymis, the coiled tube on the back of the testicle. Typically a bacterial infection travels retrograde from the urethra, prostate or vas deferens into the epididymis. It can be sexually transmitted or contamination from the rectum/GI tract. The diagnosis is often a clinical one and antibiotics will be prescribed. See your Doctor.

UTI: Symptoms are commonly seen in urinary tract infections, not as common in men, but possible. Kidney stones can sometimes cause similar symptoms, pain tends to lateralize. On the background it seems that severe constipation is contributing as well. Other possibilities cannot be excluded, he needs to be evaluated for further recommendations.

Resistance : There is becoming a higher rate of resistance to meds used for UTIs, particularly with Cipro (ciprofloxacin) and especially with Macrobid.  Keep track of your temperature and evaluate your flank pain throughout the day as you the infection could turning into pyelonephritis.  At this point, I'd recommend going on Levaquin, a stronger antibiotic.  For future simple UTIs ask for Kefex 4 x / day.

Get help for pain: There are lots of reasons for abdominal discomfort, and not all of them are due to the GI tract. Even "functional" syndromes may be present without underlying "disease." if pain with meals consider peptic (acid), biliary, or pancreatic disease. If bleeding or tenderness is present, this suggests inflammation, ischemia, or infection.  Exquisite tenderness is quite concerning. See your doctor  pls.

OMG, stop NSAID's!: There are many causes of peptic distress including: food, obesity, pregnancy, smoking, alcohol, stress, h.Pylori infection, daily or prolonged NSAID use, to name a few. Your two-year history of daily use of nsaid's (meloxicam, advil/ibuprofen) is the red flag here. Unless prescribed, stop the nsaid's, start Prilosec otc, ; assess response over 3-4 days. Call doc if persistent symptoms, bleeding...
