28 months ago i had a s/p primary, non mesh repair for a 0.5 x 0.5 cm umbilical hernia without complications during / after the procedure. due to diet / exercise have lost 20 lbs of weight. could weight loss contribute / cause recurrence of hernia?

Yes: Mesh is the best surgical option for repair of an umbilical hernia.  Hernias smaller than a centimeter can probably be repair with suture alone, but anything bigger requires mesh to decrease the risk of a recurrent hernia.

Unlikely: as the repair was done 4 Wks ago  , collagen tissue already formed which keep the mesh in place , hoe ever call your surgeon for a follow up appointmennt  do not do any heavy activity till your surgeons clears

Wait and see: do not  stretch your  abdomen too much for next 2 to 3  wks  for compete healing  then only you know if hernia is recurring .

Should not: You should not weight lift. You may cause injury again. Take up a different exercise like swimming

Scar tissue: After any surgery it is not unusual to have some scar formation. It can be on the surface or beneath the skin.

Umbilical hernia: You should be able to, but you might find the incision site to be still a bit on the tender side so you might consider waiting for at least another week or two.
