Several: Hi. The main problems with obesity are type 2 diabetes and "metabolic syndrome", a clustering of high blood pressure, bad lipids (high triglycerides, low HDL, and small dense LDL), and increased risk of atherosclerosis. If the obesity is caused by a disease (say, Cushing's or Prader-Willi), it may have it's own set of other problems related to the disease of origin.

I do not expect it: Unfortunately drug users are good at covering up their habits.  They are often charming and great manipulators.  I would not have confidence in a drug user to make wise decisions.  Be wary.

3 bili types: Please discuss this with your physican. Here is a rough ieda of normal:
normal bilirubin range in adults: 

direct bilirubin: 0.1 mg/ dl to 0.3 mg/ dl (1.7 mmol/ l to 5.1 mmol/ l)

indirect bilirubin: 0.2 mg/ dl to 0.8 mg/ dl (3.4 mmol/ l to 12.0 mmol/ l)

total bilirubin: 0.3 mg/ dl to 1.0 mg/ dl (5.1 mmol/ l to 17.0 mmol/ l).

Normal: This value is completely normal. There are a number of ways to measure total bilirubin. Either mg/dL or umol/L are the two most common. Normal total bilirubin ranges between 0.3 and 1.9 mg/dL. A bilirubin of 8 umol/L is equivalent to 0.47 mg/dL which is completely normal.
