Give it a week: it sounds to me as if your biggest problem is your anxiety, but I noticed you are not taking an SSRI class drug like Paxil, (paroxetine) Prozac or Wellbutrin. This concerns me because they work great for anxiety. Also a drug that you are taking can worsen anxiety over time, Klonepin. This class of drugs can help in the very short term, but invariably exacerbate anxiety and cause a downward spiraling effect

Not a cold sore: Cold sores occur on the outside of the mouth/borders of lips, not in the mouth and are caused by the herpes virus. Canker sores, which are not caused by the herpes virus occur in the mouth, and are typically painful. This could just be an enlarged tongue papilla, which is common and not a problem. Have your doctor check it to be sure.

Tongue cleaning: A white coating on you tongue may be caused by diet so tongue brushing is  very important.  Brush or use a tongue scraper every day. Vitamin deficiency can be another cause.
  The are medical problems that can cause a coated tongue so see your doctor if it doesn't go away or if you tongue is burning.

Sounds : Sounds like he has black hairy tongue. The bacterial makeup of his mouth has changed. Is he on antibiotics? Taking certain medications? Heavy tobacco use? Oral hygiene?

It doesn't typically require medical treatment, and its temporary.

If it has not resolved itself in a week to 10 days, see your physician for an exam.
