Many things: If his other milestones are normal then this narrows it down a bit.  We want to know details of his birth history, early life, nutrition factors, overall growth, and family history to just name a few things in order to figure this out.  Is the motor skill delay progressive? (is his muscle strength WORSENING?)  Or is it static? Could be many reasons at this point, these answers will guide diagnosis

Sure: These are movements made with small muscles of hands.  Some of these skills are pointing, using both hands to hold a toy, building a tower of two blocks, scribble with a crayon, using a spoon with some spilling, taking off their own socks.

Later Than Expected: Studies have shown that most kids follow a pattern of advancing skills regarding use of various interactional skills..There is a tremendous range of time, so a child may smile at someone as early as 1-2 mos.And as late as 5-6 mosand still be in the "normal" range. When these advancements are delayed, longer than the expected latest normal time, we worry about problems.

Peds neurologist: He should be seen by a pediatric neurologist.  You describe several areas of problems.  Some areas are perhaps doing quite well, whilst others are not.  This bears investigation.

If : If you had severe cts you may find that while your sensation improved you lost muscle tiissue. This is a common complaint as once the muscle tissue is lost from carpal tunnel it may not return or return only partially.

While sensation may also subjectivley improve, in sever cases there may be incomplete returtn and small object are still dififcult. One measure of the ability to discern small objects is called two point discrimination and your may still be elveated despite being better than before the surgery.

Other issues suchas arthritis, trigger fingers, tenindonitis can affectyour hands too and too often people think that it is all due to cts and will all go away with surgery . In these cases while surgery may partially improve a severe carpal tunnle, it doesnt stop arthritis, overuse and tendonitis.

Id talk to your surgeon about getting re evaluated for other issues.

Passing Stage!: Stranger anxiety is very common at this age; it is probably the rule rather than the exception. This is a passing stage and your toddler will eventually become more interested in exploring the world while developing independence. It is important to keep exposing your toodler to social situations rather than sheltering them from interaction. Stranger anxiety little to do with future social skills.
