That is ok if there is no pain, oozing, bleeding, itching, etc...  The causes may be irritant or allergic (something touched the area), or may be from blocked pores, blocked glands, irritated follicles, etc...  A doctor can look at the bumps to help get a diagnosis.

: The rash may be related to a change in hormone levels at birth. Neonatal acne is a common cause of the described rash. ? folliculitis: It sounds like folliculitis, a bacterial infection of the scalp similar to acne.  If you look close you may see a pustule when they first break out.  They are often  itchy/tender and scratched at producing a small scab.  They tend to come and go.  There is no permanent cure, but i often treat flares with antibiotics.  I would go see a dermatologist for a definitive diagnosis and treatment options.

Papules : Since they have been there for 4 years and has not caused a problem it  does not seem serious but you could see a dermatologist.
They sound like papules depends on the size

Spreading Red Bumps: Anything that is getting worse...Especially when it is on the skin needs to be evaluated in person.  This could be herpes. It could be a staph or fungal infection.  Please get in to see your family doctor soon.

Seborrhea?: This may well be a case of seborrheic dermatitis. The head and shouders may not help. You might want to try some selsun to see if that will help. If not call your doc for appt and advice.

Baby acne?: It's likely to be baby acne at his age, assuming that he is otherwise well and feeding normally.  Baby acne tends to occur right around this age and looks like red bumps that get redder and stand out more when baby is upset or just after a bath.  This goes away on its own in a few weeks. It is caused by maternal hormones that crossed through the placenta.  No treatment is needed.
