Extreme exercising, fasting, anorexia, can lead to irregular menstrual cycles.  Without more info too many possibilities. U should see your gyn to discuss.

Gyn/Uro symptoms: Mucus is a sign of irritation or infection. Not sure if the white material is tissue since mucus can have color at times. The urine may be picking up vaginal discharge as it is passed, and there is a good possibility that this is vaginal rather than bladder in source. Need to see a physician soon to evaluate the symptoms along with the menstrual changes. Many possibly causes for menstrual irreg.

Depends on age: Irregular cycles prior to menopause are related to irregular ovulations.  Average age of menopause is 51.5 years.
If post menopause, hot flashes normal,  bleeding should be evaluated by your doctor
