Possible: If you have pain and swelling you need to have an x-ray done .Meanwhile wear a brace and take some Aleve (naproxen)  twice a day . Schedule a visit to a hand specialist, .

PCP: You should start with your primary care doctor. They will be able to diagnose and treat most causes of thumb swelling. If not, they'll be able to send you to the correct specialist.

Thumb: Consider inflammation, sprain, strain, arthritis, gout, tendonitis.

Quite possibly: The scaphoid does not take repeated injury well and is prone to non-union. I would definitely contact your orthopedic surgeon asap.

If it doesnt improve: It's likely just a contusion or sprain, but if the pain doesn't improve by tomorrow, see your doctor for a check up.  Keep the scrapes clean.  Monitor for redness or signs of infection (drainage or pain).

Call Hand Specialist: It sounds like you should see a physician immediately. It sounds like you had a significant hyperextension injury to your thumb and you may have torn some of the ligaments that support your thumb because of the amount of swelling and redness. A doctor needs to check the blood flow to your thumb and assess any possible nerve damage,and any possible fracture/dislocation. ER if high energy injury.

Raynaud's disease: I need more info, but based on your description you could have a condition called raynaud's disease, especially if that happens after exposure to cold water and/or exposure to cold weather, this is better discussed with your regular physician..

See Hand Specialist: Impossible to tell with limited history but first thing that comes to mind is infectious process from either a bite (insects) or cut.  The timing matters, is this a chronic problem or did it happen recently?   Painful lesions on the hand usually indicate a herpetic (viral) whitlow.  So to really tell you how to fix it, you need to have an expert get a good history and physical exam.
