Strophic testis: The testicle can shrink at a very early age or later in adult life. Few of the main causes of testicular atrophy are, torsion(twisting), trauma ,infection( mumps) etc. Please see a urologist and get an ultrasound done.

i went for a sonogram of my scrotum (duplex) they entered my testicle size on the report. right testis: 4.2 cm x 1.9 cm x 2.9, left testis: 3.0 x 1.7 cm x 2.0 cm. my question are my testis small, or average? thank you?

: Right testis's size is in normal range. Your left testicle is small but etiology and its significance should be correlated with medical history. It is common to have two testes with different sizes. One normal testicle is enough for male hormone and fertility.

Mild size: asymmetry between the testicles is normal.  Also, they may normally be suspended at different angles and at different heights.  However, if you feel like there might be a problem, see your doctor for an evaluation.

See endocrinologist: What you are describing is called primary hypogonadism.  First, the cause must be identified.  Then, depending on that cause, the outlook and the proper treatment can be seen.  You need an evaluation by an endocrinologist, who can obtain the needed studies and direct our treatment

See urologist...: What described - small left testis, scrotal hanging, visible vessels - more suggests active inflammatory/infectious lesions in the scrotum, but which usually comes with no fever in a healthy man. But, for young man with fever, one has to consider epididymitis that is inside scrotum next to testis. So, see urologist  to identify what it's now if it's still in its active stage.

Will not enlarge: There is no medication which will enlarge testes. Testes will also not enlarge  any more after puberty.

Normal: Many of us are designed that way, with one testicle slightly lower than the other.  Otherwise, they'd be banging together all the time.

Becoming a man: You need to get with a competent urologist or fertility specialist, and also learn about your adult body. "Night fall" is a superstitious term used in some cultures for the normal, healthy phenomenon of nocturnal emissions, which you should be enjoying. Testes actually grow and shrink without obvious explanations. Who's doing your sperm counts, and why are you asking?

Not always...: The size of testes highly vary.  It will work well for sexual function and sperm production. If in doubt and concerning or facing the issue of infertility, see urologist and do semen analysis to check the quality of sperms. For detail, ask urologist.

Unlikely : You don't mention his testosterone level but with an fsh that high, i assume it's low. Has he had a chromosome analysis for klinefelters syndrome ? If his testes are small, whatever the cause of his low t has probably been going on for a long time making it unlikely that he makes sperm. He should get a sperm count. See a fertility specialist.
